Description
The Client Care Worker/Shelter Monitor is responsible for the direct oversight and transportation of all SMTS program participants.
 
MINIMUM QUALIFICATIONS:
 
High School Diploma or GED and at least three years of related experience.  Good verbal and written communication is a must.  A valid driver’s license and three years of driving experience is required.
 
PRINCIPAL RESPONSIBILITIES:

 
  1. Manage all direct care services including driving clients to and from appointments and programs.
  2. Encourage client’s participation in the daily routine, the recreation program and other directed activities including assisting the SMTS case managers in maintaining a prescribed scheduled.
  3. Maintain and document SMTS program participant information in the first floor SMTS log book.
  4. Monitor the main floor lobby area and supervise SMTS program participants in the sign-in/sign-out procedures.
  5. Make timely notifications to a supervisor whenever a SMTS program participant fails to comply with program protocol and rules.
  6. Ensure safety of clients, staff, building and grounds.  Search client dormitories and report and remove threats to safety such as contraband, weapons, or extension cords.  Report any residents who present a danger to themselves or those around them.
  7. Remove and inventory personal possessions of residents who have vacated the facility.
  8. Comply with relevant policies and procedures of Volunteers of America.
 
EFFECT ON END RESULTS:
  1. Clients will receive quality direct care services including transportation services.
  2. Clients will participate in the daily routine, recreation program, and planned activities.
  3. The SMTS log book will be updated with all required SMTS program participant information.
  4. The SMTS program participant sign-in/sign-out procedure will be supervised at all times.
  5. Timely notifications will be made on all clients who fail to comply with program protocol and rules.
  6. Client and staff safety is ensured and building/grounds secured.  Message of intolerance of unlawful activity is sent to residents.
  7. Vacated rooms are quickly and efficiently made available for new clients.
  8. Program will be in compliance with agency policy.
